Exciting News!

We're thrilled to announce that LabCorp is now available to draw your blood work right here in our office at Sparrow Health & Performance. This new service provides you with the convenience of completing your lab work during your visit.

If you're a new patient and would like to have labs drawn on the day of your appointment, please ensure you arrive fasting for at least 8 hours prior to your scheduled appointment. Only water is permitted during the fasting period. Otherwise, you can visit any LabCorp location or return to our office at a later time to have your blood work drawn.

For New Patients, prior to your first appointment please fill out and complete all forms, consents and personal information under My Account Details, Questionnaires and My Allergies. They must be completed at least 24 hours prior to your first appointment for adequate time to review or the appointment will need to be rescheduled. Once they are reviewed and filed by our office, your completed forms and questionnaires are shown under the Labs and Documents tab.
